Stage 2 changes (effective 1 July 2025)
Change 1 — Age-differentiated premiums.
Two age bands:
- Workers aged ≤ 50 — standard band (lower premium reflecting lower medical risk)
- Workers aged > 50 — older band (higher premium reflecting elevated risk)
Premium differential typically 30-80% between bands depending on insurer and sector.

Change 2 — Standardised allowable exclusion clauses.
MOM standardised three permitted categories of exclusions, replacing previous patchwork of insurer-specific exclusion language:
Category A — Elective / non-medically necessary:
- Cosmetic procedures
- Voluntary plastic surgery
- Treatment for fertility / reproductive choice
- Treatment for self-inflicted conditions
Category B — Treatment from employer or worker acts:
- Injuries from employer's wilful neglect
- Treatment from drug or alcohol abuse by worker
- Self-inflicted injuries
- Acts contrary to law
Category C — Medical equipment + repatriation:
- Medical equipment (e.g., wheelchairs, prosthetics) for ongoing use
- Repatriation costs (covered separately under FWMI)
Insurers cannot apply exclusions outside these three categories. This enables apples-to-apples comparison between policies and removes hidden cover gaps.
Change 3 — Direct insurer-to-hospital reimbursement.
Pre-Stage 2: Worker presents at hospital; hospital bills employer; employer pays; employer seeks reimbursement from insurer.
Stage 2: Worker presents at hospital; hospital bills insurer directly; insurer pays hospital; employer pays only co-payment portion if applicable.

MWMI must be bought from an insurer on MOM's approved Medical Insurance insurer list. Because participation changes over time, the current approved-insurer list should be checked directly with MOM at the point of procurement.
